8x8 Finds 52% Blame CIOs for AI Errors news featured image

More than half (52%) of technology leaders say CIOs are held accountable when an AI agent makes an error, according to new research commissioned by 8×8.

The survey, conducted by Censuswide in July 2026, questioned 2,501 CIOs and CTOs across the UK, US, France, Australia and the Republic of Ireland about the challenges surrounding AI adoption.

CIOs were held accountable more often than other functions, with 16% pointing to customer service leadership and 6% to legal or compliance teams.

Samuel Wilson, Chief Executive Officer at 8×8 commented:

“The CIO is now the last line of defense for AI, and in a lot of organizations, that’s a job they didn’t apply for. The CIO didn’t choose the vendor and they don’t always have the audit trail.

But when an AI agent gets it wrong, their name is the one in the incident report. That’s not sustainable, and it’s not how you build a governance model that holds up under scrutiny. The fix isn’t more oversight. It’s giving the people with the accountability the visibility to match it. ”

Data sovereignty is also emerging as a significant consideration for technology leaders. The research found that 82% said the location of AI infrastructure matters, with 30% describing it as the primary factor in vendor decisions and 52% considering it one of several critical factors.

“While many think it’s only Europeans worrying about data sovereignty, the data shows that it’s a huge issue in the US as well,” said Wilson.

“This reflects that when you are dealing with data jumping over borders and where infrastructure is located across multiple vendors and tech stacks, it’s really a problem for everyone.”

The findings follow previous research which found that 89% of technology leaders said data sovereignty had become a higher priority over the previous 12 months, amid regulatory requirements including GDPR, HIPAA and CCPA.

The research also looked at efforts to reduce the number of vendors and technologies used by organisations. Just 9% of CIOs said there was no single platform capable of meeting their organisation’s requirements.

Instead, respondents highlighted several barriers to consolidation:

  • 30% cited migration cost and complexity
  • 24% cited regulatory or data residency constraints
  • 16% cited vendor lock-in
  • 15% cited internal politics and stakeholder alignment

“The encouraging part of this research is that the technology to fix this fragmentation already exists,” Wilson added.

“What’s missing is a partner willing to help organizations navigate the politics and the change management, not just sell them another platform. That’s the harder job, and for a partner willing to step up, there are rewards to be had.”

The findings suggest that AI governance is becoming increasingly linked to communications infrastructure, particularly as organisations manage accountability, data controls and multiple technology vendors.

“CIOs aren’t short on technology options,” said Wilson. “They’re short on one view across everything they’ve already bought. Closing that gap is what will separate the organizations that get ahead with AI from the ones that just get exposed by it.”
